Housing Navigator

The Housing Navigator connects individuals and families experiencing housing instability with safe, stable housing options by conducting needs assessments, identifying applicable resources, and guiding clients through the housing search and placement process. This role builds relationships with landlords and community partners to secure housing opportunities and supports clients through barriers to housing stability.

Case Manager – Housing

The Case Manager – Housing partners with clients to develop and implement individualized service plans that promote long-term housing stability and self-sufficiency, addressing barriers such as employment, benefits, and health needs. This position provides ongoing advocacy, connects clients to community resources, monitors progress, and adjusts plans as needed to help clients achieve and maintain permanent housing.
