Transition Planning

TRAC coordinates resources and fills the gaps in planning services for youth emancipating from substitute care.
 
If you are a caseworker or care provider or a youth and would like to access TRAC services, please complete the referral form online or download it and fax it to 214.370.9305. Once we receive the information, TRAC staff will contact you within several days, to set up an interview/intake. The intake will be extensive and will last about 2 hours.
 
TRAC's Transition Services Process

 

Intake

  • TRAC Site Coordinator

  • Initial Interview

  • Gather Contact Info

  • Needs Assessment

  • Orientation to TRAC services

  • Youth transition plan worksheet

Assessment

  • WorkSource Specialist - testing; Site Coordinator - interviews youth

  • TABE locator and / or TABE

  • CHOICES Aptitude Skills

  • CHOICES Interest Profiles

  • Ansell-Casey Life Skills

  • Youth Worksheet Returned

  • Determine ITP invitees

  • Set follow up / ITP appointment

Analysis & ITP Planning

  • TRAC Staff

  • What do the tests tell us?

  • What are the most immediate needs?

  • What are some possible options?

  • “Work ready” recommendations

  • Educational / vocational options and financing

  • Immediate job needs / options

  • Housing options

  • Life skills needs

  • Support needs

Create the Individual Transition Plan (ITP)

  • Youth and their team

  • Review youth worksheet

  • Lay out staff analysis

  • Select options in each area:                                                      - Education & Training
    - Employment
    - Housing
    - Life Skills
    - Finances
    - Physical/Mental - Health
    - Community Connections
    - Relationships

  • Make a plan with timeline

  • Determine what support is needed and who will help

  • Agree on a plan for follow up
